Library Forms
The following forms are available for printing. They cannot be submitted online. You must print them out and hand them in at the library.
Interlibrary Loan (ILL)
- Please read the following conditions before you hand in your ILL form.
- Print Clearly. (If we cannot read it we cannot order it).
- Be sure to give the full bibliographic information. Do not use abbreviations in your citations. Specify the source that you obtained the information from. If you used an online database, which database did you use. Do just write "internet."
- Please allow 14 days for your materials to arrive.
- While most ILL materials are sent for free, be aware that some suppliers charge for materials. Please specify the cost you are willing to pay for materials. If you only want free materials specify "0" amount. We will call you before we order any materials that have a cost attached.
Forms are also available at the Circulation Desk. Just ask our staff and they will be happy to assist you.
